Fraser Integrated Healthcare Specialist (LMFT, LPCC, LICSW) - $2,000 Sign On Bonus! in Golden Valley, Minnesota

Fraser, a Minnesota leader in providing healthcare, housing, education, and employment services for individuals with special needs, is seeking a Mental Health Professional to collaborate with internal and external partners to support mental health screening, brief intervention and referrals as identified by Integrated Health practices. Join our multidisciplinary team! In this unique opportunity, you will get to support clients and families via telehealth from our brand new Telehub in Golden Valley. This location will support a core team of clinicians who come together and collaborate together to provide therapeutic services via teletherapy. Bilingual candidates encouraged to apply! Responsibilities: Collaborate with community partners to provide necessary supports and resources for clients and their families. Provide consultations with families regarding developmental and behavioral concerns. Collaborate with the Integrated Healthcare team to implement consultation around child development and screening, positive parenting strategies and developmental guidance information available to clinic staff. Provide consultation to medical professionals, clinical staff, including client services regarding the referral process, early childhood development, EC mental health and trauma informed practice. Provide referrals and track follow-up as appropriate to help families make connections to key resources. Document all patient clinical activity and care coordination in electronic medical records. Maintain open communication with medical staff and members of the care team. Work closely with pediatric providers around care coordination, goal setting, counseling, coaching and education about key aspects of a child's development. May facilitate parents/caregivers groups. Participate in weekly reflective supervision meetings. Fill in or provide other clinical coverage duties as requested. Actively participate in Telehealth Hub team Supervision of staff as assigned. Schedule and provide clinical services in such a way as to meet or exceed budgeted productivity targets, as applicable. Location: Golden Valley, Minnesota Schedule: Monday - Friday, standard business hours Requirements: Current licensure from the State of Minnesota (LP, LMFT, LPCC, LICSW) required. MA, MS, or Ph.D. in a mental health field required. Experience working with children & families in a clinical setting required. Experience working with individuals with developmental delays/intellectual disabilities, including autism, highly preferred. Valid Minnesota Driver's License with acceptable record required for travel to different Fraser locations for training and meetings. Ability to pass and maintain good standing in the Department of Human Services background check.
