To provide high quality, safe, compassionate patient care in an acute care setting by demonstrating clear and concise communication and collaborative skills. Works as an integral member of the care team at the direct patient level, and participates in the management of populations by identifying and bridging gaps in care.

  • Delivers nursing care under the direction of a Registered Professional Nurse and documents care delivered in the EPIC medical record.

  • Maintains Clinical Competence

  • Demonstrates a humanistic, kind, and caring attitude in the delivery of health care team and utilizes appropriate line of communication.

  • Performs nursing procedures such as vital signs, phlebotomy, and the medical administration, administration of treatments and vaccines.

  • Enters all vaccine administration data into the EPIC medical record and CIR as necessary.

  • Provides a monthly update on all vaccine inventories.

  • Answers phones and documents calls and responses in EPIC

  • Reports results and instructions to patients over the phone under the direction of the provider.

  • Assists with the preparation of forms as directed by physician

  • Checks supply inventory in the rooms, check expiration dates on all supplies and request necessary supplies on a weekly basis.

  • Keeps a manual temperature log for the vaccine refrigerator.

  • Prepares patient for surgery, diagnostic tests, treatments, and examinations.

  • Reports pertinent clinical information from phone calls and other means of communication to the provider, and provides documentation of conversations in the EPIC medical record.

  • Assists with population management; Chronic and Acute Services and Preventative services.

  • Conduct Transitional Care Management activities in accordance to rules and regulations of Medicaid

  • Participate in high risk / readmission patient quality improvement initiative.

  • Recognizes and supports the unit based model for delivery of nursing care and assumes responsibility for assignments delegated by the Registered Professional Nurse.


  • Completion of an accredited educational program for licensure as a LPN

  • New York State Licensure with current registration as an LPN

  • Current BCLS Certification

  • Knowledge of Electronic Medical Records (EMR) and basic computer
