Currently Hiring for CARE COORDINATORS in Sacramento
Care Coordinator is to provide optimal care for an entire population by identifying and eliminating care gaps. Will coordinate team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their provider.

Additional Skills & Qualifications:
Required Qualifications High School Diploma or equivalent required.
Two years related experience preferred OR equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
Excellent customer service and interpersonal skills.
Efficient time management skills, with the ability to meet deadlines and organizational needs.
Strong administrative skills with attention to detail.
Ability to maintain detailed records.
Ability to work flexible schedule when needed.
Ability to use standard office equipment including phone, fax, copier.
Computer (PC) literate in Microsoft Word, Excel, Powerpoint, Outlook email and database software.
Ability to establish and maintain positive, professional relationships.
Knowledge of patient care, addiction treatment and mental health disorders.
Knowledge of Community resources including Medicare, Medi-Cal, Supplemental Security Income (SSI) and the county’s General Assistance Program.
Knowledge of basic medical terminology and functions of medical staff services.
Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
Fluency in additional languages preferred.
Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
Ability to abide by standards of professional ethics and maintain HIPAA rules of confidentiality.
Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
Assist and support the Center/Department/Program to meet standards of High Reliability.
Must have valid driver’s license and insurable driving record
