Position description

This position ls located out of the MCHS - St James site, but will potentially serve patients in Mayo's SWMN region.The goal of the Community Health Worker is to improve health outcomes for persons within identified communities through linking identified community need with available resources to adopt healthy behaviors. CHW’s conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain and improve individual and community health. CHW will partner with existing agencies and organizations to facilitate integrated service delivery for residents of assigned community(ies). Provides information on available resources, social support, and informal counseling, advocates for individual and community health needs, and provides services in a setting where there is low potential for exposure to blood and other high risk body fluids, communicable diseases and hazardous materials, chemicals and waste. May collect data to help identify community health needs. Implements the care plan under the supervision of appropriate patient care team members per MN state requirements and reports findings to care team partners, educators, and other healthcare providers. CHWs must have demonstrated good standing within their community and an interest in improving outcomes for their neighborhood(s). Demonstrates skills in creative problem-solving, community engagement, outreach and has in-depth knowledge of the assets and challenges of the community to be served. Has the ability to work with medical professionals to ensure follow up and coordination of recommended medical and social management. Majority of time spent travelling within neighborhoods to meet with residents. Local travel required.


Completion of Community Health Care Worker certificate program from an accredited program required. Two years post-secondary education and clinical/medical experience preferred. Current, continuous and trusted member of the community. Experience with community programs or health organizations. Demonstrated skills in creative problem-solving. Ability to go door-to-door to connect with the residents of assigned community on a weekly basis. A high level of communication skills (oral and written) necessary for assessment of health concerns and referral for appropriate service delivery. Ability to effectively and professionally interact with health team members and consumers (e.g. members of the community). Physical ability to perform all assigned duties; able to stand and walk approximately 75% of the work time. Knowledge of the needs of low-income families. Sensitivity to cultural diversity and varying coping styles of families under stress. Ability to work independently. Population specific experience preferred. Knowledge of population characteristics and desired outcomes. Experience working in a team environment. Computer skills (e.g. PowerPoint, Excel, Word) required prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making. Also aware of cultural communications and legal and ethical implications to providing care. Strong human relation skills are needed to interact with patients, patient’s family members, consultants, residents, hospital personnel, community agencies, and other resources. Must have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and written form. Must possess skills in conducting individual, group, and family education sessions, including engaging in supportive counseling and therapeutic interventions with patients.

Additional qualifications Qualified candidates must be bilingual in English and Spanish.

License or certification

Must have a valid driver’s license.

Completion of Community Health Care Worker certificate program from an accredited program required. If completion / graduation did not occur prior to hire, then enrollment of accredited community health worker program must occur within 3 months of hire and completion of a maximum time period within 2 years. Certification or degree from accredited Community Health worker program. Basic Life Support (BLS) is not required but preferred. If providing face to face services across state lines, licensure/certification for applicable states is required.

Compensation Detail

Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $19.79 per hour.

Most hours are M-F 8am to 5pm, however, expect to work flexible hours as needed including days, evenings, and weekends.
