Responsibilities

Seeking an ACT Substance Abuse Specialist to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Healthcare Clinic in Tempe, AZ.

Full-Time Employed: 40 hours/week - 4 10's schedule

Location: Priest Drive and Broadway Rd. in Tempe, AZ

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

$2/hour ACT Differential

This role provides direct treatment, rehabilitative and supportive services in a community setting to clients served on the ACT team in the Recovery Clinic. Works as a team member of all team to assess member needs and wants, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies. Serves as the ACT Team’s specialist for substance abuse issues by providing other team staff with information and knowledge in respect to substance use issues. Provides specific substance abuse interventions to clients as clinically indicated. Provides on-call crisis intervention services, as well as weekend and holiday coverage.

ACT Substance Abuse Specialist duties include:

  • Provides time-unlimited, community-based services to all clients served on the ACT team.

  • Takes full responsibility for the treatment services for clients; coordinates care with other ACT team members and other providers to ensure client needs are met and that the client continues to receive treatment in the community to the greatest extent possible.

  • Utilizes assertive engagement mechanisms, including street outreach and legal mechanisms (probation/parole, court ordered treatment), when necessary to re-engage clients in prescribed treatment.

  • Is responsible for crisis services for all ACT team clients, coordinating hospital admissions and discharge planning.

  • Provides all services to clients in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.

  • Works with clinical team to assess client needs, develop individual treatment plan, provide direct care, coordinate services across providers, continuously evaluate client needs and functioning, and advocate for client needs in accordance with case management best practices and regulatory guidelines.

  • Facilitates substance abuse supports, either individually or in groups, per clients’ individual service plan.

Qualifications

  • Education/Experience:

  • GED/High school diploma with a minimum of 3 years of behavioral health experience or,

  • An associates degree in the field of behavioral, or

  • A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or

  • A bachelor's degree in the field of behavioral, or

  • A master's degree or higher in the field of behavioral health

  • Bachelors Degree in the field of behavioral health or an equivalent combination of experience and education

  • Minimum of three (3) years experience in the delivery of substance abuse services required

  • Strong understanding of substance use, addiction and recovery clinical skills

  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members

  • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience and meet requirements of Terros Health’s driving policy

  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment

  • Must understand and utilize a recovery approach and philosophy

  • Exemplify great customer service and motivational skills

  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions

  • Must be able to work on a computer and tablet in Microsoft Office, with electronic medical records and in NextGen

  • Provide services in a clinical setting and in the community, as well as weekend and holiday coverage
