Serves as a liaison between Mercy Health Youngstown (MHY) and the community. Assists with defined aspects of a community health program including teaching about basic health issues. Identifies health related issues affecting the community, collects data, and informs MHY of community needs. Encourages and facilitates access to health and social services. Able to work independently.

The Community Health Worker (CHW) serves as a liaison to the target community and is primarily responsible for facilitating access to health and social services, teaching about basic health issues, and assisting with needs assessment. Assists with data collection and tracking.

ESSENTIAL FUNCTIONS (7-10 statements)

% Time

60%Encourages and facilitates access to health and social services through a range of activities such as outreach, social support and advocacy. Connects community members to resources such as health and social services, health insurance, food, housing, education, career and job training, and fitness and wellness opportunities. May assist clients with scheduling appointments, registration and filling out insurance and financial aid forms. Responsible for following up with patients to confirm they connected with the referred resouces.

15%Collaboratation with care team physicians/LSW/LISW

10%Accurately collects and tracks data through Epic for program and grant reporting. Maintains good record keeping of all activities. Competent in the use of computer, Microsoft Office, Word, Power Point, Excel and Access for tracking purposes.

5%Develops and maintains positive working relationships with community members and co-workers.

5%Travel between offices as needed.

5%Assists with other duties as directed by Coordinator, Supervisor, or Manager.

Required Minimum Education: High School or equiv

Specialty/Major: Competence in writing and reading.

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Required: Certified Community Health Worker by the Ohio Board of Nursing
