The Health Care Coordinator (HCC) position has primary responsibility for clinical oversight and management of resident care during their shift of operation. The HCC shall be a professionally licensed nurse (RN or LPN) and shall exercise clinical judgement in emergency and routine situations within the constraints of the Nurse Practice Act. Additional responsibilities include contributing professional clinical expertise for the health promotion program for residents.This program includes the sessions for residents which promote self-care and management of chronic diseases and disease prevention. Focusing on increased self care for residents, the HCC is a resource and teacher for Resident Coordinators, Resident Assistants and residents. The HCC works cooperatively with interdisciplinary staff and in conjunction with the Case Manager, Program Manager and Associate Director. The HCC is responsible to the Case Manager, PCHome Administrator or the Healthcare Manager of Passavant Retirement Community for the performance of this position. Qualifications Graduate of RN or LPN-approved nursing school with current PA license. Minimum of 2 years practical application of professional skills in either med-surg environment or working with frail elderly persons. Working knowledge of common chronic conditions prevalent in the elderly population, as well as commonly prescribed medications/protocols and potential interactions and adverse reactions. Effective with group process; demonstrated supervisory skills, leadership, delegations, and evaluation of caregivers. Passes competency testing.
